Speech and/or Language Evaluation

The first step in receiving speech or language services is to complete the evaluation.  This will include, but not limited, to the following:

• Family History 

• Client/child history 

• Assessment with SLP 

• Reviewing the results of the Evaluation

If you click on "Forms", you will see the forms to download, complete, and email before or during the evaluation/session.  During the assessment session(s), an evaluation for speech or language will be completed with your child based on your (the parent/guardian) concerns.

As the parent/guardian, you will receive a copy of the evaluation results for your records.
