Child Intake Form: Complete to provide your concerns and child/family history

Consent for Services: Your signed consent for evaluation and services to be implemented

Acknowledge of HIPAA Privacy Notice: When we start discussing evaluation and clinical services, you will receive a document outlining HIPAA Privacy as it pertains to SLP services.

Communication Preferences: Your signed consent for how you would prefer to communicate with the clinician.

There will be other forms to review and complete/sign once the evaluation is scheduled. These forms will be emailed to you, including General Acknowledgement of Forms, Attendance Cancelation Policy, Payment Policy.
